A new model of health care delivery is set to begin in Ottawa with the announcement of the city's first health team.

"Together with our health care partners, the Eastern York Region North Durham Ontario Health Team will play an essential role in delivering on our commitment to end hallway health care and building a connected and sustainable public health care system centred around the needs of patients", shared Elliott.
Eventually, home and community care functions will transfer to Ontario health teams, said Hayley Chazan, spokesperson for Elliott.

Among the many facets of health care, the health team has thus far identified mental-health and addictions services as one of several priorities in its first year of collaboration - an area that, if improved, could have one of the largest impacts on the health-care system and the broader community as a whole.

"The tangible thing that's coming is intangible - collecting information about what we're doing really well in this region, where do we have space to improve, and bringing the sector stakeholders together to say, 'Well, if these are the areas we could improve in, are there ways we can work together and in our own sectors apart in trying to make those improvements?" So, for now, LHINs will continue to be in charge of home care. Its goals are keeping patients as healthy as possible in their communities and out of hospitals, providing the right care in the right place, improved patient flow to ensure that no one is lost in the system and families are being overlooked and to build up their patient care capacity.
